Community Connections

A 10-week community skills and independence program for individuals ages 14 and up with disabilities in Durham Region.

Community Connections is a 10-week program for individuals with disabilities aged 14 and older designed to promote skill development, confidence, and community engagement. Participants develop practical skills such as community navigation, money management, public transit awareness, and functions skills while taking part in weekly experiential learning opportunities (trips).

Weekly experiential learning takes place at a variety of community locations , including grocery stores, restaurants, the library, and other local destinations. With support available as needed from staff members, participants regularly use public transit to travel to and from these locations. These experiences provide opportunities to practice navigating the community, build familiarity with public transportation, and develop greater confidence in everyday settings.

A strong focus of Community Connections is transit awareness. Participants regularly use public transit to explore destinations across the community including grocery stores, restaurants, libraries, and more, building the independence and confidence to get around on their own terms.

Who is Community Connections for?

Individuals ages 14 and up who identify as having a disability in Durham Region, and who want to gain confidence navigating the community  .
